• Completed transportation application, including historical TIV
• Commodities broken down by percentage
• Drivers list that includes date of hire and years of experience for all drivers
• Equipment list that includes VINs for all equipment – units that are 16-20 years require a certified mechanics inspection
• Currently valued loss runs for 3 years. Account size and market dependent.
• MVRs that include documentation proof of prior 2 year USA issued Class A CDL and copy of commercial driver’s license
• Valid DOT number
• Insured Tax ID number
