Become a Producer

Thank you for your interest in working with AmWINS Transportation Underwriters. In order to initiate the appointment process, we ask that you complete our
Agency Questionnaire and provide us with a copy of your E&O dec page, resident and non-resident business entity licenses, resident and non-resident individual producer licenses, and a copy of your most recent balance sheet and income statement.

All of the information to start the process has been assembled into a PDF document, available here. Once you open the document, you simply need to complete, sign, and fax the forms back along with the requested information.

Please note that the Agency Questionnaire forms are fillable and can be completed online and then printed.

For your convenience, we have included a pre-filled fax coversheet. Please attach the required information and fax back to us using the attached coversheet.
